Sick notes, forms and physicals: what they cost and how long they take
4 min read
Paperwork is one of the most common reasons people visit a clinic — and one of the most common sources of frustration when expectations are not set.
Sick notes
A physician can only certify what they have assessed, which means a note usually requires a visit. Ontario has limited when employers may demand notes for short absences, so it is worth asking your employer whether one is genuinely required before you book.
Notes are uninsured. The fee is posted and payable at the time of the visit.
Insurance, disability and school forms
Drop the form at reception rather than bringing it to an appointment — most require a chart review rather than an exam. Typical turnaround is one to two weeks, longer for detailed disability forms.
Complete your portion first. Incomplete patient sections are the single biggest cause of delay.
Physicals and medicals
- Driver's medicals, including commercial and senior renewals
- Pre-employment and pre-placement physicals
- Camp, school and sports participation forms
- Immigration-adjacent forms where applicable
How to avoid surprises
Ask the front desk for the fee and the expected turnaround when you drop off the form. Keep the receipt — many employers and insurers reimburse the cost, and some will pay the clinic directly if you ask them to.
